The only number to contact the surgery is 01604 709922, any previous numbers are no longer in use.

On Wednesday 5th December we will be closing for training at 12.30 hrs and not re-opening until 08.00 hrs on Thursday 6th December. If you require medical advice during this time, please telephone 111. Thank you.

Flu season is well underway at the surgery, if you are entitled to a vaccine and have not yet had, please contact Reception at the surgery to book, before we run out!

We are now registering new patients at the surgery, please speak to Reception for further details, or see our section new patients.

SystmOnline

Online System

This service is available 365 days a year, 24 hours a day.  You can:

  • Book, cancel or amend an appointment;
  • View test results including history from when you registered;
  • Order repeat prescriptions;
  • Access your summary care record;
  • Proxy access for your children under 13 (GDPR regulations).

How to register for the Online Services?

To access the service you will need to download the following form or collect from Reception:

Please bring the completed form and 2 proofs of ID ( 1 of which needs to be photo ID, such as a passport or driving licence) to Reception.  Once these have been approved you will be issued with a username and password sent to you via email.

Unfortunately, patients aged 13 to 16 years are unable to have online access.  Proxy access can be given to a parent / guardian for children aged 12 years and younger, and this will automatically cease once the child reaches 13 years old.
