It is important we are able to communicate with our patients.  For us to do this, we need to have up to date contact details, we need:

• Home address
• Telephone number
• Mobile number
• Email address

By having up to date contact details, this allows us to send out confirmations and reminders for appointments and communicate with you if we need to for any reason.

For all patients over the age of 16, we need your details and not of your parent.

Telephone calls are recorded for training and quality purposes.
