Welcome to Dr C Moore & Partners, Wootton Medical Centre (WMC). There are approximately 7000 registered patients, cared for by an effective, dedicated, and comprehensive primary healthcare team, which includes GPs, Practice Nurses, Clinical Pharmacist, Social Prescriber & Reception / Administration team.
We are part of the ParkWood Primary Care Network (PCN) working collaboratively with Denton Village Surgery and The Parks Medical Practice.
We are a small team with an open & honest ethos, everybody is valued & opinions are heard.

Telephone Calls To & From WMC:
We are duty bound to make patients aware that incoming and outgoing telephone calls are automatically recorded. This is done for a variety of reasons, mainly to help with staff training and quality purposes.
Wootton Medical Centre Team:
In order to provide the best service in a challenging time for the NHS we have a mix of health care professionals that make up our Primary Care Team.
Care Navigators - our Reception team are trained to help navigate you to the most appropriate member of our health care team, which is not always the Doctor. All our staff abide by the same professional and confidentiality standards. They will ask you brief details about the reason you are requesting an appointment.
Practice Nurses - are able to deal with a wide variety of issues including long-term health conditions such as diabetes, asthma and COPD, cervical screening (smears), pill checks, injections and travel.
Health Care Assistant - is able to deal with blood tests, ECGs, blood pressures, dressings, suture removal and NHS Health Checks.
Phlebotomist - is able to deal with blood tests.
Clinical Pharmacist - improving value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks.
Social Prescriber - taking a holistic approach to people's health and wellbeing, they connect people to community groups and statutory services for practical and emotional support.
GPs - our GPs are the core of our Primary Care Team who are able to manage general medical and health problems.
