Wootton Medical Centre is currently closed to new patients due to maintaining current patient services and managing an ever increasing workload.  This is for a temporary basis and we will update our New Patients section when our situation changes.

The only number to contact the surgery is 01604 709922, any previous numbers are no longer in use.

We are offering DoctorLink at the surgery, please see below and section - Appointments & Clinics - for more details.  You may be directed to this when asking for an on the day appointment.

In November, 67 patients failed to attend or cancel their appointment at Wootton Medical Centre, please help us to help you by cancelling if you no longer need this appointment as these can then be offered to another patient.  See Appointments & Clinics section for more details.

Due to maintaining current patient services, which we do not want to compromise on, and managing an ever increasing workload, Dr C Moore & Partners, have taken the decision not to register new patients at Wootton Medical Centre on a temporary basis, until further notice.

Our website will be kept up to date when our situation changes.

NHS England and Nene CCG are aware of this decision.

To Register at WMC


We are accepting new patients at Wootton Medical Centre but you must live within our practice catchment area of Wootton, Quinton and Hardingstone.

To register, you will need to complete a GMS1 form and our registration form:

GMS1

New Patient Registration (Adult)

New Patient Registration (Child)

Please print and complete these forms, bringing them to Reception with the following ID (everybody over the age of 16 will need to register in person):

  • 1 x proof of identify, such as passport, driving licence or birth certificate, and
  • 1 x proof of address. such as a bank statement or utility bill, dated within the last 3 months
