142 patients have booked but failed to attend or cancel their appointment. In February 2020, 78 patients failed to attend or cancel their appointment at Wootton Medical Centre, please help us to help you by cancelling if you no longer need this appointment as these can then be offered to another patient.  See Appointments & Clinics section for more details.

New Patient Registration

We are accepting new patients at Wootton Medical Centre but you must live within our practice catchment area of Wootton, Quinton and Hardingstone.

To register, you will need to complete a GMS1 form and our registration form:

Until further notice, please complete the above documents and send to along with copies of your ID.  We are NOT accepting registrations in the surgery.

Please allow up to 72 hours for the process to take place.  If you are on regular medication, please ensure you have 28 days supply from your previous practice.

Please print and complete these forms, bringing them to Reception with the following ID (everybody over the age of 16 will need to register in person):

  • 1 x proof of identify, such as passport, driving licence or birth certificate, and
  • 1 x proof of address. such as a bank statement or utility bill, dated within the last 3 months
