Sick / Fit Note Certificates
Self certification form for less than 7 days
If you have been ill for 7 days or less you do not need to see a GP to obtain a fitness to work certificate (sick note). You can complete a self-certification form yourself. Many Many employers have their own self-certification form. If your employer does not, you can download a self-certification form by clicking on the pdf document below.
Some employers may insist on a Doctor’s note regardless, and if this is your situation, there may be a charge.
Fit Note for more than 7 days
If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates (fit note).
You can request a fit note from Reception. Your GP may call you or ask, via Reception, that you attend an appointment.
If you have already been seen by your GP about this illness your certificate will be available to collect after 48 working hours.
During covid-19, all certificates will be sent via email.
Requesting an additional fit note
If you have already had a fit note for the same illness your GP may not need to see you to issue an additional fit note. Please request this via Reception.
If we do not make any further contact with you, your fit noe will be available to collect after 48 working hours.
