NHS England introduced Summary Care Records (SCR), which will be used in emergency care. The record contains information about any medication you are taking, allergies you suffer from and any bad reactions to medicines you have had, to ensure those caring for you have enough information to treat you safely.
No other information will be included in the SCR.

You can change your mind at any time by informing us of your decision. If you have previously told us that you do not wish to have a SCR, your preference will already have been recorded on your medical record.
