NHS England introduced Summary Care Records (SCR), which will be used in emergency care. The record contains information about any medication you are taking, allergies you suffer from and any bad reactions to medicines you have had, to ensure those caring for you have enough information to treat you safely.
No other information will be included in the SCR.
As a patient, you have a choice:
- I would like a SCR – you do not need to do anything, the SCR will automatically be created for you.
- I do not want a SCR – you need to complete an opt-out form (PDF).
You can change your mind at any time by informing us of your decision. If you have previously told us that you do not wish to have a SCR, your preference will already have been recorded on your medical record.
A dedicated NHS Summary Care Record information line is available to you on 0300 123 3020 or you can visit their website.
